Our Lady of Guadalupe Catholic School strives for excellence in the health and safety of our students, faculty, staff, parents, and visitors. We are blessed to be part of the Christus Health School Clinics Network. Our clinic is staffed with a medical assistant two days a week and a clinic coordinator three days a week. Our goal is to maintain a safe environment for our students.
Every student enrolled in a Catholic school in the state of Texas must be immunized against vaccine-preventable diseases caused by infectious agents in accordance with the immunization schedule adopted by the Texas Department of State Health Services. A student who fails to present the required evidence shall not be accepted for enrollment. The only exception to the requirement is a medical exemption signed by a licensed physician (M.D. or D.O.) authorized to practice in the State of Texas.
Vision and hearing screenings are conducted annually as mandated by the State of Texas.
Forms that should be completed for students with special medical conditions are listed below. A medication permission form can be used for PRN or daily medications (such as ADHD medications). If the student with asthma is older or in after-school sports the parent should have both the “Asthma Action Plan” and the “Asthma action plan for self-carry” forms completed.

To extend the healing ministry of Jesus Christ by providing primary healthcare services to children, faculty and staff in the school setting. Our primary goal is to help keep the kids healthy and keep parents at work.
Family Nurse Practitioners are the primary care providers in the clinics. The clinics are overseen by a physician who serves as the Medical Director.
Nurse Practitioners (NPs) provide high-quality healthcare services similar to those of a doctor. We can diagnose and treat most common illnesses and prescribe medications. NPs also focus on health promotion and disease prevention.
